The law says that you should be involved as much as possible. Your views, needs and wishes should sit at the heart of the plan.
Everyone must listen to your views and feelings, your dreams and goals and the support you need to achieve them.
Have you got a One Page Profile?
A One Page Profile has all the important information about a person on a single sheet of paper under three simple headings:
what people like about me,
what’s important to me
and how best to support me.
A One Page Profile can be a great way of sharing with school or others the most important information about yourself.

Have you thought about a vocational profile?
A vocational profile is a document that can be used as way of helping you to plan what work you would like to do in the future.
It is a way of putting together your thoughts:
- what you like and don’t like.
- It can help you to choose what you want to do.
- It is a helpful way to share your views.
Your vocational profile should be done with help from a trusted adult who can help you to discuss your choices and fill in a form. It might be someone from school or college or a young person’s worker.
The form will include things like:
- What you enjoy and what you are good at.
- What you would like to do when you are older.
- What is important to you,
- What kind of support helps you best.
Your vocational profile should be kept up to date with new information like if you do some work experience or your views might change.
You should take your vocational profile with you when you leave school or college.
If you have an EHC plan you can ask for a summary of your vocational profile to be attached to it.
